Index Out of Range on App Wizard

Issue
An error occurs while going through the App Wizard on K2 Platform Classic.
Index was out of range. Must be non-negative and less than the size of the collection. Parameter name: index
Symptoms
Steps to reproduce the error are as follows:
- Open K2 Workspace and navigate to Apps.
- Click on Create a New App if no App has been created; otherwise, click on New... from the toolbar.
- Provide a Name for the App.
- For the Template, choose Employee Leave Request.
- Click on the button Next (Primary Detail).
Resolution
Please install at least K2 Platform Classic (5.4) Fix Pack 2 to get the fix for this issue.
