Issue

When attempting to edit a View or a Form from the artifact page, you receive a "An item with the same key has already been added" error.

If another user attempts to check out said form, they encounter an error stating that the original user has it checked out even though the object appears already checked in.

Symptoms

Check the [Form].[Form_Design] and the [Form].[Form] table to see if the View or Form in question is checked out to the user. See if there is a mismatch, such as it being checked out in [Form].[Form_Design] but checked in in the [Form].[Form] table.

Troubleshooting Steps

If there is a mismatch, you will need to force check in the View or Form with the issue.
