Issue

Task notifications and Email events for a workflow do not have the SharePoint Attachment that was added in the Form. 

Symptoms

There are no Attachments in the Email, however the attachment does show on the List itself.

Troubleshooting Steps

First, check how the Workflow is being started. 

If it is being started by a SharePoint event (Item added, updated, etc.), you should not have to worry about this. Just make sure that the Attachments Item Reference exists. 

If it is started by SmartForm rule:
Check that the Item Reference for the Attachments is being added in the Start a Workflow rule.
